Geriatric Care Management, LLC (GCM,
LLC) located in Albuquerque, New Mexico is
an agency comprised of a team of professionals
who specialize
in
assisting
older people and their families with long
term care arrangements. Working with family
members or directly with
the elderly client, our geriatric care managers
conduct a comprehensive assessment to determine
the options that best fit the individual.
Our fees are private pay. We do have two discount programs based on either proof of limited assets and income or paying ahead, via retainer. Most Long Term Care Insurance policies will also cover payment for our assessments and some of our continuing care management services.

Geriatric Care Management recognizes, values, and respects the unique qualities of each aging person, evaluates their needs, and locates the best resources to maximize the person’s level of independence, while honoring individual wishes and values so that the most informed decision will be made.
GCM enhances decision-making, choice, and affordable solutions, in determining and obtaining the type of care that is needed and stays involved in order to coordinate care on a continuing basis, making adjustments as needed, and avoiding crises.
About the Staff
GCM, LLC is owned by Martha Brown (Marty). Because Marty is licensed as an Independent Social Worker (LISW) by the New Mexico Board of Social Work Examiners, she is licensed to practice independently. She is a member of the National Association of Social Workers (NASW), the National Association of Professional Geriatric Care Managers (NAPGCM) and also Chair of the 2007 Annual Conference for the Western Region Chapter of NAPGCM. As a member of these professional organizations, Marty adheres to high standards of practice and ethics. Also, Marty is a Registered Guardian and member of the National and New Mexico Guardianship Associations.
She received her Master’s degree in Gerontological Social Work in 1976 from the University of Chicago and is bi-lingual in Spanish. Marty has participated in several initiatives aimed at educating the community on the issues of aging. As part of this effort, she has conducted workshops and given presentations for professionals, families, and health care organizations as well as presented at local, national and international conferences.
Marty employs social workers, gerontologists, certified care managers, registered nurse consultants, registered guardians, and other health care professionals as needed to ensure a high quality of individualized relationship with each client. The staff ensures availability and advocacy in a time of need. The overall goal of GCM involvement is to help each client maintain or achieve his or her highest level of independence.
